In order for insurance to be properly processed we required accurate billing information for your group health, workmans' compensation, and personal injury. Please provide us with a contact person and phone number as well as any other pertinent information necessary to process your claim. Please be aware that insurance is submitted weekly, provided that we are supplied with all the accurate required billing information.

Our Office provides the courtesy of calling your insurance company to inquire about your insurance benefits. However, the insurance company states that a quote of benefits is not a guarantee of payment.

• It is your responsibility to know and understand your insurance plan. If you have questions about your coverage, please seek assistance.
• To find out insurance coverage, call the phone number on the back of your insurance card.
• You will be responsible for the portion of charges not covered by your insurance plan.
• Ostir Chiropractic Clinic is now accepting assignment from Medicare.
